What are your hours?
Our business office hours are Monday through Friday; 9:00 am - 4:00 pm. We offer therapy appointments outside of these hours based on each individual therapist schedule.
How do I schedule an appointment?
Scheduling an appointment can be done one of two ways. You can either call our intake line at 704.438.9901 or you may email us at email@example.com. The choice is up to you!
How do I reschedule an appointment?
If you are a new client, please contact the business office at 704.438.9901 to cancel or reschedule your appointment. If you are an existing client, please contact your therapist directly.
What information do I need to schedule?
We need basic information to start: your name, date of birth, address, insurance information and any accommodations you may need.
Do I need to do anything after I schedule my appointment?
When you schedule your appointment you will be asked to complete the necessary paperwork below prior to your appointment. If you are unable to do this please arrive 20 minutes early and we will have the forms waiting for you.
What will I owe at the time of service?
We do our very best to verify your insurance and determine if you have a required deductible or co-payment. At times, we are given incorrect information and you may be responsible for additional payment once a claim is submitted.

We are an out-of-network provider for all other insurance companies. You will be given a receipt for payment and you may personally choose to file a claim with your provider.
Fees are payable at the time of your appointment by cash, visa, mastercard or Health Savings Account.
We require 24 hours notice for cancellation of a session. This is to allow the time to offer the appointment to another client who may be on a cancellation list waiting for an appointment. A $60 fee will be charged for non-attendance or less than 24 hours notice of cancellation.
